The inspector’s account

On December 29, 2025, Licensing Program Analyst (LPA) Murial Han conducted a case management - other visit to confirm staff # 1 (S1) is not working at the facility. LPA met with the administrator and explained the purpose of today's visit.

During today's visit, LPA toured the facility with the administrator who confirmed that S1 does not work at the facility.

Based on the facility's monthly master work calendar for August, Septometer, October, November and December of 2025 and the LIC 500 (Personnel Report), LPA observed S1 was not on the schedule.

This report is reviewed and discussed with the administrator.
