Facility Evaluation Report
On May 05, 2025, at 9:45 AM, Licensing Program Analyst (LPA) Kiran Jain arrived unannounced at the facility to conduct a Case Management – Incident visit regarding an incident that occurred on April 28, 2025 when three (3) residents (R1-R3) reported bug bite marks to the staff. Upon arrival, LPA met with the Director of Admissions (DOA) Michelle Ly. The LPA disclosed the purpose of the visit.
DOA stated that 1 of 3 residents (R1), who lived in room #1 on the 1 st floor got bitten by a bed bug and 2 of 3 residents (R2 and R3), who lived on the second-floor room got bitten by carpet beetle bugs. These incidents happened on Monday, April 28, 2025. The facility scheduled an earliest appointment with the Western exterminator, who came on Wednesday, April 30, 2025. The facility staff showed the bugs to the Western exterminator technician and technician confirmed the bed bug and carpet beetle bugs.
DOA stated that the whole house got treated on April 30, 2025. The technician sprayed the whole house, bottom of the mattresses and in-cased every pillow, mattress and boxsprings. The Western Exterminator is coming back in 2 weeks for the follow-up visit. DOA further stated they had been vacuuming and cleaning everyday and one of the residents found 1 dead bed bug. The bed in room #1 got treated and the next step would be to replace this bed. DOA stated there were 2 beds in room #1. A Canine Detection Company come for inspection on April 17, 2025, and after that visit the bed where bud bug was detected in room #1 was replaced on April 19, 2025. Prior to that, the Terminix performed bed bug treatment on March 25, 2025, when all the beds, furniture, and base boards in the entire facility were treated.
LPA reviewed the Canine detection report, dated 04/17/2025. According to the report, a live bed bug was found under the boxspring in the downstairs room. A recommendation was made to re-treat the room #1 as well as common areas such as living room.
