Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin arrived at the facility unannounced to

conduct a case management – other visit. LPA met with Administrator (ADM) Aida Urena. LPA stated the purpose of the visit.

The purpose of the visit is to hand deliver an immediate exclusion letter for an individual (S1) who the

Department determined engaged in conduct inimical.

ADM stated S1 no longer works in the facility. ADM stated S1 was in the process of being hired in January 2024, but S1 was not hired due to not completing the on-boarding process. ADM states S1 did not work in the facility at anytime.

The immediate exclusion letter was handed to the ADM. ADM was informed to remove S1 from any contact with clients and not allow S1 to be physically present in the facility. ADM was advised to separate S1 from the facility roster. LPA requested a copy of the updated LIC 500 to be emailed to CCL by 3/11/2026.

No deficiencies were cited per California Code of Regulations, Title 22.
