Facility Evaluation Report

Licensing Program Analysts (LPAs) Maria (Mita) Partoza and Coryse Ngangmenyi conducted an unannounced case-management-incident visit regarding additional medication errors reported to the Department on 07/30/2026 and 08/04/2026. LPAs met with Administrator (ADM) Kathleen Alcala, Medication Room Manager (MRM) Sheenal Hernandez and Assistant Administrator (AADM) Lisa Gulliani. LPAs stated the purpose of the visit.

The Department conducted additional interviews and requested for copies of staff training, staff communication, procedures when doing a medication pass for prescribed medications and as needed (PRN) medication. LPAs requested for 1 of the 2 residents' physician's report, appraisal needs and services plan, medication administration report (MAR), centrally stored medication and destruction record (CSMDR).

LPAs requested for ADM to submit their plan of action and training plan for staff who are assigned to do medication pass.

At this time, the case is under review. The Department will conduct a follow-up visit, if warranted.

No deficiencies were cited based on California Code of Regulation (CCR) Title 22 and a copy of the report was provided to ADM Kathleen Alcala.
