Facility Evaluation Report

Licensing Program Analyst (LPA) Maria “Mita” Partoza conducted an unannounced case-management-incident visit regarding medication errors reported to the Department. LPA met with Administrator (ADM) Kathleen Alcala and stated the purpose of the visit.

The Department received incident reports on 07/15/2026, and 07/20/2026 regarding prescribed medications that were not provided to clients in care. During the visit, LPA interviewed facility management and obtained incident reports, medication administration records, physician orders, staff schedules, medication-related documentation, corrective-action records and interviewed 2 staff.

At this time, the case is under review. The Department will conduct a follow-up visit, if warranted.

No deficiencies were cited based on California Code of Regulation (CCR) Title 22 and a copy of the report was provided to ADM Kathleen Alcala.
