Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct a case management - other visit to follow-up on the shower renovations. LPA met with Assistant Administrator (AA), Armando Guba.

During visit, LPA toured the facility with AA and the Assistant Maintenance Supervisor and observed construction in progress in the shower room between room 224 and 234. There are posters and tape posted on the closed doors that are currently off-limits during construction. The project is estimated to be completed by July - August 2025.

Construction hours are Monday - Friday from 7:00am - 3:00pm. Rooms 224, 232, 235, 233, 229, and 331 are off limits during these hours. It was stated that residents residing in these rooms are awake and out of their rooms by 7:00am, and after 3:00pm they are able to access their rooms safely. Regular programming to include activities, meals/dining, laundry, and medications are not affected.

Rooms 224 and 232 are currently vacant and the 4 residents in these rooms are temporarily relocated to the multi-purpose room until the project is completed. LPA observed the multi-purpose room and observed 4 beds, linens, 4 dressers, 4 night stands, and adequate lighting. LPA advised to place a curtain to cover the sliding door looking into the courtyard for the resident's privacy.
