Facility Evaluation Report
Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit regarding an SOC341 that was received in our office on 01/06/26. LPA announced the purpose of the visit and Staff Training Coordinator (STC) Justin Williams. Designated Administrator Felicia Lehner was unavailable due to illness.
STC stated that the facility had a meeting with R1s Care Manager, Program Coordinator, Program Manager and Adminsitrator to address incident. The facility will be conducting a meeting and future training that will be ongoing from 01/29/26. The facility is working with R1s care team to address behavior and coming up with a plan to redirect and concerns with R1s toileting needs.
During visit LPA toured the 3 bathrooms and 4 bedrooms and observed the facility was clean and no foul smell in the bedrooms or bathrooms of residents.
LPA interviewed 4 staff. LPA reviewed 3 resident files and observed an incident report for R1 that was not submitted to licensing dated 01/01/26. LPA explained the need for incident reports to be reported to the department. LPA provided email and fax number to STC to submit future incident reports.
Deficiency was cited during today's visit per California Code of Regulations Title 22 and a repeat violation civil penalty of $250 dollars is being issued. See LIC 809-D and LIC421-FC.
An exit interview was conducted with STC Justin Williams and a signed copy of this report and appeal right were provided.
