Facility Evaluation Report

Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Plan of Correction (POC) visit to follow up on deficiencies issued on 12/11/2025. LPA met with Administrator (ADM) Orlando Ferrer. LPA stated the purpose of the visit.

The facility was cited the following Type A and B deficiencies on December 11, 2025:

80087(g ) , Buildings and Grounds, POC due date December 12, 2025--Type A

80075(b)(6)(D) Health Related Services, POC due date December 12, 2025---Type A

80061(a)(b) Reporting Requirements, POC due date December 18, 2025---Type B

LPA conducted a Case Management visit on 1/2/2026 and observed toxics and chemicals were locked and inaccessible to clients in care. LPA also reviewed staff training conducted on 12/15/2025 and 12/18/2025 to include but not limited to medication training and safely storing chemicals.

A Letter of Deficiencies Citations Cleared was provided during visit.

No deficiencies cited during today's visit per California Code of Regulations, Title 22. An exit interview was conducted with Administrator (ADM) Orlando Ferrer and a signed copy of this report was provided.
