The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced case management visit to record the time and date LPA visited the facility and amend the complaint report submitted on 05/08/2026.

LPA met with Staff/Registered Nurse (RN) Eduardo Lizardo and stated the purpose of the visit. Administrator Jocelyn Canosa was not available during the visit due to prior commitment. Administrator was contacted by phone, and have given the authorization to Staff RN to sign for the amended reports via email.

LPA amended the complaint report with reference #26-AS-20251231134530, removed identifying information.

No deficiencies were cited and a copy of the report was provided to Staff (RN) Eduardo Lizardo.
