Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced case management visit to record the time and date LPA visited the facility and amend the complaint report submitted on 05/08/2026. LPA met with administrator (ADM) Jocelyn Canosa and stated the purpose of the visit.
LPA amended the complaint report with reference #26-AS-20251231134530, removed identifying information and, added new information that was received by the Department.
No deficiencies were cited and a copy of the report was provided to ADM Jocelyn Canosa.
