Facility Evaluation Report

Licensing Program Analyst (LPA) Marcela Yanez conducted an unannounced case management visit following an incident where resident AWOL'd from the facility. LPA announced the purpose of the visit and met with Administrator Matthew Miao.

During visit LPA toured the facility and interviewed 3 staff including the Clinical Nurse, Program Specialist and Administrator.

ADM stated the facility filed a missing persons report as a protocol. R1 has not returned to the facility and has not been located. The facility did have 2 staff on shift when R1 had left the facility.

No deficiencies were cited per California Code of Regulations, Title 22.
