Facility Evaluation Report

Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Administrator Ray Mirelez.

The purpose of the visit was to follow up on an incident report submitted by the facility on 08/21/2025. The incident report started that on 08/21/2025, an outside party reported to the facility that a staff hit resident R1. The staff was not identified in the report. The facility administrator interviewed five staff, and all five staff denied observing a staff hit R1. The administrator observed R1 and did not observe any bruising or signs of abuse.

During visit on 09/26/2025, LPA Marrufo interviewed ten staff. All interviewed staff denied observing a staff hit R1. All interviewed staff denied having hit R1. All interviewed staff denied having observed any bruising or signs of abuse on R1.

LPA Marrufo conducted a telephone interview with R1’s family member, FM1. FM1 stated to conduct body checks on R1 each day R1 returns home from the facility. FM1 stated to have not observed any signs of abuse on R1.

No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Administrator Ray Mirelez and a copy of this report was provided.
