Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM)Janessa Flores.

LPA observed 2 staff and 2 resident in the facility. The other 2 residents went to day program. LPA checked 2 resident record files and 2 staff record files. ADM stated the facility is a level 7 facility. LPA toured the facility inside and out with ADM. Facility license, Administrator Certificate , and Personal Rights posters were observed posted at the facility.

Living room, family room, kitchen, dinning area and three restrooms were inspected. 4 resident bedrooms, and garage were inspected. 1 staff room was observed in the facility. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. The temperature of the refrigerator was observed at 37 degree F and the temperature of the freezer was observed at 0 degree F. Medication closet, knives closet and Dish cleaning product cabinet were observed locked. Room temperature was at 74 degree F, and hot water temperature was at 109 degree F in facility. First Aid box, flash lights, and night lights were observed in the facility. The facility has emergency lighting system. The last time the facility conducted the emergency drill was on 5/8/2026.

Fire extinguisher was serviced on 01/26/2026. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff, and were working. Front yard and backyard were inspected. There was no obstruction to block the walkways. .

No citation noted today. Exit interview was conducted with ADM. This report was provided to ADM for review and for signature.
