The inspector’s account

LPA Steve Chang toured the facility and made the following observations: LPA observed two bathrooms were clean and in sanitary condition. LPA checked the refrigerator and did not see expired food in the refrigerator.

On June 4, 2025, LPA Chang and Rai conducted a follow up complaint investigation visit. LPA’s Chang and Rai attempted to interview residents R1-R2. Both residents did not respond to questions posed by LPA’s. Both residents ignored LPA’s questions.

LPA’s Chang and Rai interviewed staff S2 and S3. Both staff interviewed stated they clean the facility everyday.

LPA Chang and Rai made the following observations during the complaint investigation visit: kitchen, common area, 2 bathrooms and 3 bedrooms to be in good repair and sanitary conditions.

On June 20, 2025, LPA Manuel Monter toured the facility inside and out, including but not limited to: kitchen, dinning area, living room, resident bedrooms, bathrooms. LPA was observed to be clean and sanitary.

LPA Monter interviewed Staff S4. S4 he/she cleans the facility during his/her shift. S4 stated he/she hasn't seen the facility in a dirty, unclean state.

Based on investigation, records reviewed, and interviews conducted, the Department found that the above allegations are UNSUBSTANTIATED . An unsubstantiated finding indicates that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the allegations did or did not occur.

Facility staff are locking residents in their bedroom.

On April 22, 2025, the Department received a complaint alleging, Facility staff are locking residents in their bedroom.
