Complaint Investigation Report

On November 20, 2025, LPA Manuel Monter interviewed Program Director Melissa Ramos. PD stated a week ago facility staff was cooking with a cooking stove and propane tank. PD stated that day the facility was making pancakes. PD stated when they were cooking in the storage area, they had the roll up door up. PD stated this only occurred 1 time. PD acknowledged that cooking in the storage area was unsafe. PD stated they have removed the propane tank and cooking stove from the facility.

The Department reviewed Local Fire Department report dated July 10, 2025 and November 13, 2025. Both reports stated that it was observed that the inside of the warehouse of the facility had a cooking stove and propane tank inside.

Based on interviews and documents review the preponderance of evidence standard has been met therefore the above allegations is found to be SUBSTANTIATED.

Deficiencies are being cited, per Title 22 code of regulations, see LIC9099-D. This report was reviewed with Program Director Melissa Ramos. Appeal rights were provided.
