Facility Evaluation Report

Licensing Program Analyst (LPA) Manuel Monter arrived unannounced to deliver the results of a complaint investigation. During the complaint investigation, a case management visit was conducted due to a technical violation discovered during the investigation process. LPA met with Program Director Melissa Ramos .

While investigating the complaint 26-AS-20240126142223 , Staff S1 admitted that client C1 tried to hit C2 and S1 held C1's waist from behind to try to hold him/her back to prevent the altercation.

LPA discussed with Program Director that manual restrain should not be used unless an emergency intervention plan is developed and approved by the Department prior to the use of such techniques. ADM agreed and understood.

A technical violation was cited during todays visit. This report was reviewed with Program Director Melissa Ramos, and a copy of the report was provided.
