Facility Evaluation Report

On March 28, 2024, Licensing Program Analyst (LPA) Manuel Monter, conducted an unannounced case management-Incident visit regarding an incident report on March, 28, 2024 . LPA introduced himself and explained the purpose of the visit.

On March 27, 2024, the Department was informed by the administrator of alleged abuse regarding staff S1 and client, C1. ADM reported that she was informed by another staff, of alleged physical abuse. ADM stated it was alleged, that staff S1 punched client C1 on the face.

On March 28, 2024, the Department received an incident report, that stated at 9:45am, on March 27, staff S2 informed ADM that staff S1 had punched C1 in the face. At 2:00pm, ADM spoke with S1. ADM stated during facility training, ADM heard S1 ask S2 why he/she reported her to the ADM. ADM stated she heard a lot of yelling, and there was no clients present at the time.

LPA interviewed 6 staff (S3-S8), the ADM and 1 client (C1). ADM stated after she was informed of the alleged abuse, she assessed the client and did observed any marks or bruising.

ADM stated her plan of action regarding staff arguing in the day program, is to suspend the two staff S1 and S2 indefinitely while she investigates the allegations. ADM stated its her policy to let the staff go if there is argument or altercation (verbal/physical) between staff.

No deficiencies cited during todays visit. A copy of the report was provided.
