On 7/1/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the Annual 1-year required inspection. LPA Calandra was greeted by Joni Tsukimura, Director of Assisted Living and Andrii Skyrypkar, Health Services Administrator and explained the purpose of the visit.
LPA reviewed 5 resident files. All were observed to be complete.
The Annual will be completed at a later date.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report was provided.
