On 9/15/2025, LPA Grace Donato conducted a Case Management-Other visit to the facility. LPA met with Assisted Living Director Joni Tsukimura and explained the purpose of the visit.
The visit is regarding about the stipulation order sent to the facility and to confirm that the staff member (S1) mentioned in the order is no longer working in the facility.
