The inspector’s account

Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct the required 1-year annual inspection. LPA met with Assistant Administrator (AA), Triponia "Pony" Tuazon.

The day program is community based and no clients observed on-site. During visit, LPA toured the day program to include the activity rooms and common bathrooms. All exits routes were free and clear of obstruction. Facility has an approved waiver to have the hot water temperature maintained at over 120 degrees F as the building is shared with other offices. Warning signs posted at every sink. Hot water temperature measured at 114.6 degrees F. Bathroom is shared with other tenants in the building wherein the clients require supervision to the bathroom by staff. Facility temperature maintained at 72 degrees F. Fire extinguisher last services on 06/03/2025. Carbon monoxide observed operable. Chemicals and disinfectants observed locked. AA states they do not handle any client medications. LPA reviewed 3 client records were maintained and complete. LPA reviewed 3 staff files were maintained and complete. Staff files that were reviewed obtains a fingerprint clearance, health screening, TB result, and updated 1st Aid certification. Staff are provided annual training on topics related to their job to include infection control. Facility has an infection control plan. PPE supplies at the entrance observed to include gloves, masks, and hand sanitizer. Facility has an updated emergency disaster plan. Emergency drills are completed quarterly and the last drill was completed on 07/19/2025. LPA observed flashlights and batteries. Facility has a complete first aid kit. Posters observed to include COVID-19 information, personal rights, if you see something say something poster, and facility license.

No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Assistant Administrator, Triponia "Pony" Tuazon and a copy of the report was provided.
