Facility Evaluation Report
Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to complete the annual inspection focusing on infection control. LPA met with Assistant Administrator, Triponia "Pony" Tuazon and Administrator, Sandy Zipagan. The day program is community based and no clients observed on-site.
During visit, LPA toured the day program to include the activity rooms, isolation room, kitchen, common bathroom, and transportation vehicle. All exits routes were free and clear of obstruction.
The day program has a central entry point to include symptom screening and temperature check for all staff and visitors. Hand sanitizer made available at entry and throughout the program. Bathrooms observed to be supplied with hygiene products and paper supplies. LPA observed supply of Personal Protective Equipment (PPE) supplies. Donning and Doffing PPE sign posted next to isolation room. Staff clean and disinfect multiple times daily and as needed. Staff have not completed N95 fit-testing. LPA observed the transportation vehicle to be up to date with maintenance and supplied with sanitizer and disinfectant spray. LPA reviewed the facility's procedures to include screening, isolation, disinfecting, staffing, training, PPE usage and social distancing.
The following signs observed to include special visitors, visitation policy, symptoms of COVID, feeling ill, social distancing, and mask required.
No citations were issued per the California Code of Regulations, Title 22. Advisory note provided.
