Facility Evaluation Report
Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with Administrator (ADM) Elaine Yalung.
LPA observed 3 staff and 5 resident in the facility. LPA checked 3 resident record files and 3 staff record files.
LPA toured the facility inside and out with ADM. Facility license, Administrator Certificate, and Personal Rights posters were observed posted at the facility.
Family room, living room, kitchen, dinning room and two restrooms were inspected. 4 resident bedrooms, laundry room, and garage were inspected. 1 staff room was observed in the facility. Two day perishable food supplies and seven day nonperishable food supplies were observed sufficient. The temperature of the refrigerator was at 38 degree Fand the temperature was at 0 degree F. Medication closet, knives closet and dish cleaning product cabinet were observed locked. Room temperature was at 72 degree F, and hot water temperature was at 113 degree F in facility. First Aid box, flash lights were observed in the facility. The last time the facility conducted the emergency drill was on 4/10/2026.
Fire extinguisher was serviced on 08/25/20256. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff, and were working fine. The facility has emergency lighting system. All the exit doors of the facility have door alarms. Front yard and backyard were inspected. There was no obstruction to block the walkways. One storage room was observed in the backyard.
No citation noted today. Exit interview was conducted with ADM. This report was provided to ADM for signature.
