Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct a case management – legal/non-compliance visit. LPA met with Administrator (ADM) Debbie Silva. ADM states the facility has 48 clients, with 20 direct support staff. The purpose of the visit is to ensure the facility is adhering to the compliance plan submitted to Community Care Licensing (CCL) after a non-compliance meeting held on 12/12/2024.
During visit, LPA toured the interior and exterior of the facility with ADM to include the office, staff break room, music and media (computers, games, books for clients) rooms, and facility transportation vans. LPA inspected 4 facility transportation vans with ADM, with 2 vans having a wheelchair lift. LPA observed each van with the wheelchair lift, has wheelchair safety procedures posted on the back door. ADM states the 2 vans were non-operational due to required maintenance/repairs. ADM states the facility has 10 additional transportation vehicles (8 SVS vehicles- 6 ambulatory and 2 for wheelchairs, and 2 rentals for work groups). Facility vehicle maintenance logs were also reviewed.
LPA reviewed staff training records to include but not limited to topics of: RBT training and Transport (6/3/2026), Wheelchair training (6/12/2026), RBT Training-Zero Tolerance/Client Abuse (6/24/2026), Transportation training (7/22/2026). ADM states staff were provided Crisis Prevention Institute (CPI) training in March 2025 and training is valid for 2 years. LPA also reviewed Staff Meeting records from May 2026 to July 2026 to include van evacuations.
The ADM was advised regarding the importance of adhering to the facility's compliance plan developed on 12/12/2024 to ensure compliance with Title 22 regulations. No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Administrator (ADM) Debbie Silva and a copy of the report was provided.
