The inspector’s account

Licensing Program Analyst (LPA) Maria (Mita) Partoza conducted an unannounced case management – legal/non-compliance visit and met by program director (PD) Debbie Silva and Regional Director (RD) Daniela Verar.

The facility is licensed to serve 60 adults ages 18 and over, of which 30 may be non-ambulatory. License is subject to the condition of the fire clearance. The program operate from M-F 9:30 AM 3:00 PM. The facility had the annual required inspection on 04/06/2026, and no deficiencies were cited during the annual inspection.

The purpose of today's visit is to ensure the facility is adhering to the compliance plan submitted to Community Care Licensing (CCL) after a non-compliance meeting held on 12/12/2024. During visit, LPA inspected ambulatory and non-ambulatory transportation vans. LPA conducted an interior inspection of 1 wheelchair vehicle van with the RD and PD and observed wheelchair safety procedures and diagrams posted on the back door, LPA verified that the van is compliant with emergency kit, such as fire extinguisher, hazard cone, and first aid kits.

LPA randomly interviewed 3 staff and stated they are provided in-service training on wheelchair loading and unloading and vehicle safety and accident procedures and when to call 911, when to report the incident and has knowledge of mandated reporting form. Staff are aware of client rights and are currently in conducting Registered Behaviorist Training for case managers. LPA conducted a random inspection of the staff file record and verified that staff are have fingerprint and criminal background record, with current training certificate on file.
