Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection and met with Administrator (ADM) Debbie Silva.

LPA checked 5 client files and 5 staff files.

LPA toured the facility with ADM including offices, quiet room, 4 restrooms, kitchen and dining room, Art room, client locker room, storage rooms, staff break room, class rooms, copy room and conference room.

Room temperature was observed at 70 degree F. Dish washing solution closet and knives closet was observed locked. The facility has emergency light system. The facility fire extinguishers last serviced was on 12/02/2025. First Aid boxes were observed in the facility. The last time the facility conducted the emergency drill was on 2/6/2026.

The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Carbon monoxide detectors were tested by staff, and were functional.

The facility does not have backyard. There is a parking lot in front of the facility. There were no obstruction to block the walkway to the parking lot and exit to the street at the back of the facility.

No citation was noted for today visit. Exit interview was conducted with ADM. The report was provided to ADM for review and for signature. A copy of the report was provided to ADM.
