Facility Evaluation Report
Licensing Program Analyst (LPA) David Marrufo conducted an unannounced Case Management visit and met with Aida Urena.
The purpose of the visit was to respond to two incidents self-reported by the facility via Unusual Incident/Injury Report (IR). The first IR was reported on 06/05/2026. The IR stated that on 06/04/2026 at approximately 8:35 PM, staff S1's romantic partner entered the facility without authorization and began to physically assault S2. During the incident, resident R1 was present and observed S1's romantic partner physically assault S2. Staff called law enforcement and police officers arrived at the facility.
The second IR was reported on 06/12/2026. The IR stated that on 06/11/2026, S2 and S3 had taken two residents each in their own separate vehicles to the beach. Prior to departing the beach, S2 was observed to walk unsteadily. S3 offered to take the residents from R2's vehicle and drive them to the facility, but S2 declined. While driving back to the facility, S3 could observe S2's vehicle weaving in and out of traffic lanes. At approximately 7:45 PM, police pulled over S2's vehicle and put S2 into custody. Staff made arrangements to transport the two residents who had been in S2s vehicle back to the facility. No residents were harmed or injured during the incident.
During visit, LPA Marrufo interviewed two staff, one resident, and Administrator Aida Urena. LPA Marrufo obtained copies of facility records. No deficiencies were cited at this time as per California Code of Regulations Title 22. This report was reviewed with Administrator Aida Urena and a copy of this report was provided.
