Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Staff S1 Lyn Pena. During the visit, LPA observed 1 residents and 2 staff. LPA explained the purpose of the visit.
LPA toured the facility inside out with S1 which included the Living room, kitchen, dining room, 3 restrooms and 4 residents bedrooms. The front yard and backyard were inspected. There was no obstruction to block the walkways. LPA observed a storage shed in the backyard, being used as a storage space.
While touring the home, LPA observed facility Living room, located adjacent to family room and garage. LPA observed the living room was converted into two staff bedrooms. LPA spoke to ADM Valera via phone call. ADM stated the living room has been converted for years. LPA pointed out the facility sketch on CDSS file doesn't match. LPA pointed out the facility sketch that is hanging in the hallway between bedroom 1 and bathroom 1, has the living room listed as a living room. Inside the "living room", LPA observed two staff bedrooms, with a walls and doors. LPA asked ADM if the facility had gotten the living room approved to be used as a living space by the city or fire department. ADM stated she didn't know and would follow up with the licensee.
