Facility Evaluation Report
Licensing Program Analyst (LPA) Manuel Monter conducted an unannounced annual inspection visit, and met with Administrator Audrey Ann Salvador. During the visit, LPA observed 1 residents and 3 staff. LPA explained the purpose of the visit. Monte Bello Adult Care Home is a level 6 home.
LPA toured the facility inside out with ADM which included the Living room, kitchen, dining room, 4 restrooms and 5 residents bedrooms. There was no obstruction to block the walkways. The staff area of the facility was also inspected. The front yard and backyard were inspected. LPA inspected facility garage, which is being used for a storage area.
LPA toured the facility staff room. The facility sketch shows the staff bedroom, as an open bedroom. During today's visit, LPA observed the staff room contained a wall, with 2 doors, which have been constructed. LPA informed ADM that this added wall and doors did not reflect on the facility sketch. LPA spoke to Licensee (LN) Teodoro, via phone call. LN stated she isn't sure if the changes in the staff bedroom have a permit. LN stated she would send an updated fire clearance request to CDSS for the changes currently in the staff room.
Two-day perishable food supplies and seven day nonperishable food supplies were observed. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. Room temperature was at 70 degrees F, and hot water temperature was measured at 118 degrees F in resident bathrooms.
