The inspector’s account

Licensing Program Analyst (LPA) Christine Kabariti arrived unannounced to conduct a case management - incident visit. LPA met with Administrator, Audrey Ann Salvador.

The purpose of the visit is to follow-up on an incident report received for resident (R1) who attempted to leave the facility on 09/27/2025 at approximately 1:00pm. Based on R1's physician's report, R1 is not able to leave the facility unassisted. On the incident report, it was stated that a staff left R1 within a line of sight and followed R1 as he/she walked across the street. After five minutes of staff following R1, R1 returned back to the facility. It was reported that R1 was upset because he/she was not picked up by family due to the family being COVID-19 positive. R1 also communicated to staff that he/she was upset that he/she wanted to go to the recycling center to get money for snacks and a Halloween costume for his/her sister. Staff explained to R1 that R1 did not have enough recycling material yet to convert it to money, but R1 did not understand as R1 has not recycled before. This resulted in R1's leaving to across the street to look for more cans and bottles to recycle. After the incident, R1 behaviorist was contacted who will conduct a training to mitigate situations where R1 will be frustrated and wish to elope. Training was completed with staff lead the behaviorist on 10/01/2025.

During today's visit, the staff (S1) who followed R1 across the street was interviewed. S1 stated to have always had a line of sight of R1 and R1 went back to the facility within 10 minutes after looking for cans and bottles across the street in the apartment complex.

No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Administrator, Audrey Ann Salvador and a copy of the report was provided.
