The inspector’s account

Licensing Program Analyst (LPA) Christine Dolores arrived unannounced to conduct the facility's annual inspection focusing on infection control. LPA met with lead staff, Toribia "Belen" Piscasio and Administrator, Tayebeh Sedigh.

During visit, LPA toured the facility with lead staff to include the kitchen, activity room, living room, resident rooms, staff rooms, bathrooms, garage, and backyard. Facility temperature was maintained between 66 - 68 degrees Fahrenheit.

All staff present are fingerprint cleared and associated to the facility. Staff were not observed wearing a face mask.

Facility has a designated entry point for sign-in, temperature check, and verbal screening for COVID-19. Hand sanitizer made available at entry. Bathrooms supplied with paper supplies, hygiene products, and hand washing sings. LPA observed the facility's Personal Protective Equipment (PPE) supplies and PPE cart located next to the isolation room. Facility has multiple trash bin with lids. Facility has a designated isolation room and has procedures to isolation. Staff clean and disinfect multiple times daily and as needed. Staff are trained on infection control. Staff are currently N95 fit tested. The following posters observed to include cough etiquette, keep facility clean, donning and doffing PPE, and symptoms of COVID.

At 11:47AM, LPA observed an emergency exit in a resident's room blocked by a bed and night-stand. Facility staff states they were cleaning and immediately moved the furniture to clear the exit.

No deficiencies were cited per California Code of Regulations, Title 22. Advisory notes provided. This report was reviewed with Administrator, Tayebeh Sedigh and a copy of the report was provided.
