Facility Evaluation Report
Licensing Program Analyst (LPA) Christine (Dolores) Kabariti arrived unannounced to conduct the facility's required - 1 year annual inspection. LPA met with Administrator, Naomi Apostol - Miles.
During visit, LPA toured the day program with ADM to include the activity rooms (yellow, orange, green, and purple room), 2 bathrooms, kitchen, office, and exterior. All emergency exit routes were free and clear of obstruction. No participants observed on-site and were out in the community. The leak in the purple room observed repaired. ADM states the small hole on the ceiling will be patched the next week. Facility temperature maintained at 68 degrees F. Bathroom hot water temperature maintained at 111 degrees F. Refrigerator temperature maintained at 36 degrees F. Freezer temperature maintained at 0 degrees F. Clients food is brought in by the client and prepared at their home. Fire extinguisher last serviced on 04/03/2025. Facility has operable carbon monoxide detectors. Emergency grab and go backpacks observed near the entrance. Facility has extra flashlights. Chemicals observed locked. Facility has a complete first aid kits and PPE supplies. No medication is currently being administered in the facility but the facility has a locked cabinet for medications if needed. Emergency drills are completed quarterly and the last drill was on 06/16/2025. Facility has an updated emergency disaster plan and infection control plan.
5 client records and 5 staff records were reviewed. Resident records are updated, complete and signed. Staff records are complete to include annual training logs, first aid certification, health screening and TB results.
No deficiencies were cited per California Code of Regulations, Title 22. This report was reviewed with Administrator, Naomi Apostol-Miles and a copy of the report was provided.
