The inspector’s account

On 4/16/2026 LPA Grace Donato conducted an unannounced annual visit to the facility. LPA met with

Remedios Bose who arrived around 10:21am and explained the purpose of the visit.

LPA toured the facility inside and outside. While touring the facility it was observed that the temperature was at 69 deg F. Hot water was also tested and temperature was 108 deg F. Resident bedrooms observed well maintained with beds, linens, dressers, night stand and adequate lighting. Showers equipped with non-slip mats. Sharps and toxic materials were observed locked and inaccessible to residents. Carbon monoxide monitor is working properly. All fire extinguishers have been checked and was last serviced on 5/2/2025. Residents has their own grab and go bags supplied with multiple items not limited to food and water, first aid kit, blanket, flash lights, and radio. There is also adequate amount of food, 2 days for perishables and & 7 days non-perishable. Sharps and toxic materials are locked. Emergency drills are done quarterly.

Four resident records and three staff records were reviewed. Medication review was done and all medications

are accounted for and centrally stored medication records are updated. All staff has criminal record clearance

and are associated with the facility. Resident records are checked and all are complete and updated. PNI money was counted and found to be complete.

Report is reviewed with Administrator and a copy is provided.
