The inspector’s account

Licensing Program Analysts (LPAs) Marcella Tarin and Manuel Monter arrived unannounced and met with Staff S1 Arlene Backeng. S1 contacted Administrator (ADM) Justin Williams via phone and ADM did not answer. LPA contacted ADM via phone and left a voicemail stating the purpose of the visit. The purpose of the visit is to follow up on the facility's pending Plan of Correction (POC) issued during a Case Management visit on 4/24/2025.

On 4/24/2025, LPA Tarin conducted a Case Management visit regarding bedbugs. During visit, LPA observed that the facility was being treated by pest control for bedbugs. LPA also reviewed facility incident reports and observed that the f acility did not report to the Department treatment for bedbugs on 2/27/2025. The facility submitted an incident report for treatment of bedbugs to the Department on 4/16/2025. LPA Tarin issued two deficiencies and a Plan of Action was developed with the ADM.. The POCs were due on 4/25/2025 and 5/1/2025. The Department received the POCs by their due dates.

LPAs toured the facility to include 6 clients bedrooms. LPAs inspected client mattresses, bedding and furniture. LPAs did not observe any bedbugs.

LPA Tarin cleared the deficiencies cited on 4/24/2025 during today's visit. A Letter of Deficiency Citations Cleared was printed and provided to S1 during today's visit.
