Facility Evaluation Report
Licensing Program Analyst (LPA) Marcella Tarin arrived unannounced to conduct the facility's Required-1 year annual inspection and met with Licensee Marlene Elliott. LPA stated the purpose of the visit. There were no clients present during the inspection. Licensee stated all 4 clients were at day program.
LPA toured the interior and exterior of the facility to include but not limited to the kitchen, dining area, living room, 3 client rooms and 1 client bathroom. All exits and passageways were free and clear of obstruction.
LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days.
The facility was equipped with smoke and carbon monoxide detectors. All smoke detectors functioned properly when tested by Licensee. Fire extinguishers were last serviced on 5/14/2024. LPA reviewed the facility first aid kit, and it was observed to be complete. The facility emergency drill log was reviewed. The facility's last drill was on 3/1/2025. Drills are being conducted monthly.
LPA toured 3 client rooms. 3 out of 3 client rooms have a bed, functioning lights, dresser/table, bedding and space for personal belongings. LPA toured 1 client bathroom.
