The inspector’s account

Licensing Program Analyst (LPA) Melisa Rankin conducted an unannounced plan of correction (POC) case management visit to the facility above. The reason for this visit was to conduct a follow-up in reference to deficiencies that were cited during a case management visit that occurred on 04/15/2025. The LPA met with Ingrid Chacon, Team Lead Unlicensed, at 11:27 AM and explained the purpose of the visit. LPA informed Team Lead that the POC is now considered late.

On 4/15/25 facility was cited for Title 22 Section 81061(j) The licensee shall notify the licensing agency, in writing, within 10 working days of a change of administrator or program director. On 4/16/25 LPA Rankin emailed Gloria Torres a copy of the Case Management Report from 4/15/25, which shows the POC, the regulations cited, and the due date of 04/30/25.

On 5/1/25 facility submitted LIC 308 Designation of Facility Responsibility via email to LPA. In which LPA responded on 5/1/25 that per the Plan of Correction all required documents were to be submitted to comply with 81061(j) which included 81061(j)(3) “Description of his/her background and qualifications, including documentation of required education and related experience.” On 5/5/25 LPA Rankin sent a follow-up email to confirm that Gloria received the request for additional documentation to comply with the Plan of Correction, on 5/6/25 Gloria responded confirming receipt of email.

As of today, the facility did not provide the LPA with the required documentation within the appropriate time frame. As this is a failure to correct the POC within the given time frame and no extension was requested, a civil penalty is being assessed in the form of $900. The total of the civil penalty is calculated at $100 per day starting after the due date of the POC. The POC for this deficiency was due on 04/30/2025 and today’s date is 05/09/2025. The Team Lead was also advised, $100 per day will continue to accrue until the Plan of Correction has been cleared.

Exit interview conducted, civil penalty assessed, appeal rights provided, and a copy of the report delivered.
