Facility Evaluation Report
Licensing Program Analyst (LPA) Melisa Rankin conducted an unannounced CASE MANAGEMENT visit to this facility in conjunction with a complaint. The purpose of this Case Management visit is to issue a citation for a deficiency observed during the course of the complaint investigation that is not directly related to the complaint allegation.
Upon arrival LPA asked to speak with Administrator on file. LPA was told by staff that Administrator is no longer here. LPA spoke with new administrator Gloria Torres over the phone and asked when the change occurred, Administrator stated the change occurred in November of 2024. LPA stated the update to Community Care Licensing (CCL) had not been made, Administrator responded she was aware and they did not know that it had not been updated as of approximately two (2) weeks ago. LPA stated this will result in a citation due to the length of time and no attempt has been made to update the record.
Per the CCR, Title 22, Division 6, Chapter 2, the following deficiency is cited: 81061 (j) The licensee shall notify the licensing agency, in writing, within 10 working days of a change of administrator or program director.
