Facility Evaluation Report

Licensing Program Analyst (LPA) Melisa Rankin arrived at 10:02 a.m. to complete the 1-year required annual visit. LPA met with Alexandria Neal, Team Lead Unlicensed.

LPA interviewed additional staff and 6 clients.

Staffing: The facility currently employs 9 staff. Files reviewed had current 1st Aid/CPR Personnel Records/Application. LPA reviewed staff training. Facility utilizes Relias for assignment of and record keeping of staff training. Interview with staff, they would like more in-person topic-specific training especially in regard to the various mental health support their clientele need.

Disaster Preparedness: The current emergency disaster forms were provided and will be posted. The facility last conducted a quarterly disaster drill/training in August 2025. Emergency exits and telephone numbers were posted. A set of keys is available for staff on all shifts to access full facility in an emergency.
