The inspector’s account

At 9:00am on 05/08/2025, Licensing Program Analyst (LPA) Jeffries arrived to the facility unannounced to conduct the facility annual inspection. LPA met with Administrator, Rea Bonner Hallmon, announced who he is as the reason for the visit. LPA also issued final findings on two separates complaints during this visit.

This facility has 4 bedrooms, 3 bathrooms, staff office, kitchen with a chef's pantry, dining room, an expansive 3 section living room, and an activities room. there is a garage on south east end of the facility that is locked and used for storage and there is a small medications room that is locked. There is a covered patio in the back yard with shade and seating for clients and visitors. Each room is single client occupancy. Laundry room is in the hallway of rooms one and two. There are three full first aide kits, with main kit located in the medication closet and all are complete per regulations. Administrator tested smoke alarms/carbon monoxide detectors that are all hardwired throughout the facility and in working condition. LPA noted that all client rooms met regulation requirements including furniture and linin requirements. LPA noted three fire extinguishers throughout that facility all primed and in the green. LPA observed at least 2 days of perishable foods and 7 days of non perishable foods on hand for 4 clients and staff. LPA noted that the facility is clean and in good repair with no exits or pathways blocked.

Administrator and LPA reviewed Emergency Disaster and Evacuation Plan (LIC610D), Infection Control Plan (LIC9282), Client files, Staff files, and Medication Administration Records (MAR) for completeness, with no issues. LPA conducted a full review of the annual care tools modules and found no violations or citations. At this time there are no violations or citations issued as a result of this annual inspection. Exit interview, report read, and report provided.
