The inspector’s account

QA and LPA made observations of the food on hand at the facility and noted that there were at least 2 days of perishable foods and at least 7 days of non-perishable foods on hand for 4 clients and staff. LPA noted that the same specific observations have been made and noted in past reports. At this time, there is not enough evidence to support the allegation of, “Staff does not ensure clients have appropriate amounts of food.” and is unsubstantiated at this time.

As to the allegation of, “Staff do not treat clients with dignity and respect.” It was alleged that the facility is a hostile environment and clients are always sent to their rooms. It was discovered through interviews and observations that on 08/04/2025, QA and LPA conducted interviews in 4 of 4 clients residing at the facility. C1, C2, C3, and C4 all stated, “staff do treat us with dignity and respect.”; “no problems with any of the current staff.”; “never been threatened by any current staff.” ;“staff do not make problems in the home.”; and they are,” in bedrooms at my choice not staff request.” On 08/04/2025, QA and LPA conducted interviews of S1, S2, S3, and S4, all stated they have had client rights training, and that they treat clients with dignity and respect at all times. All staff stated that they have not observed other staff treat clients inappropriately. LPA noted that QA and LPA have observed facility staff working appropriately with clients in the facility on current and past visits. At this time there is not enough evidence to support the allegation of, “Staff do not treat clients with dignity and respect” and is unsubstantiated at this time.
