The inspector’s account

All three clients stated that all staff treat them with dignity and respect. All three clients stated they have the choice to stay home or go on outings depending on the staffing. On 04/17/2025, C1 stated that on a resent outing they had a disagreement with Staff 2 (S2) but that was initiated by C1 (self admitted by C1) and that staff treats every client the same respect and dignity. On 04/17/2025, QA and LPA conducted interviews with S1, S2, S3, and S4. All four staff indicated that they have had personal rights training at this facility. All four staff said all staff are respectful of the clients in care. All four staff denied any staff or administrators “playing favorites” with any of the clients in care. At this time there is not enough evidence to support the allegation of, “Staff do not treat clients with dignity and respect.” and is unsubstantiated at this time.

As to the allegation of, “Licensee does not ensure staff are adequately trained.” It was alleged that S5 can not read or write and is not equipped to pass medications. It was discovered through documentation and interviews that on 04/17/2025, QA and LPA conducted interviews with S1, S2, S3, and S4. All four staff indicated that they have had personal rights training at this facility. All four staff stated that they have had training from this facility passing medications. On 04/17/2025, QA and LPA conducted interview with Administrator Rea Bonner Hallmon, who stated that all staff get medication training, LPA review training documents that verified all staff have medication training. Additionally, Administrator stated that all employes hired are able to read and right, and they do provide alternative language training if a staff member a English as a second language and is more comfortable with Spanish. On 04/14/2025, LPA Jeffries received and reviewed staff training logs and documentation indicating 8 hours of training for current staff. LPA observed S5 on those training records with signatures. At this time there in not enough evidence to support the allegation of, “Licensee does not ensure staff are adequately trained.” and is unsubstantiated at this time.

As to the allegation of, “Facility is not providing clients in care with adequate amounts of food.” It was alleged that, Food is always an issue barely enough food for clients. It was discovered through observations and interviews that on 04/17/2025. QA and LPA conducted interviews with C1, C2, and C3. All three clients stated that they always have food and free food choice. All three clients stated they have never had any issues with the food amounts or quality at this facility. On 04/17/2025 QA and LPA conducted interviews with S1, S2, S3, and S4. All four staff stated that there is always adequate amounts of food in the facility and they have never run out or been short of food for the clients. On 02/24/2025, 03/04/2025, 04/17/2025, and 05/08/2025 LPA had observed facility food inventory and found no issues with food amount or quality and to be meeting the regulatory requirements. At this time there is not enough evidence to support the allegation of, Facility is not providing clients in care with adequate amounts of food.” and is unsubstantiated at this time.
