Facility Evaluation Report

At 11:00am on 03/11/2026 Licensing Program Analyst (LPA) Jeffries arrived to the facility unannounced to conduct the facility annual inspection visit. LPA met with Administrators Assistant, Valerie Yeabrra, announced who he is and explained the purpose of today's visit. Program Manager Kelly Eatton, joined the inspection on cell phone.

Administrator Assistant and LPA toured facility inside and out. Hot water temperature measured within regulations of 105*-120*(f). All toilets and hand-washing facilities are maintained in a safe, sanitary, operating condition. The program site is clean, safe, sanitary and in good repair for the safety and well-being of clients, employees, and visitors. Facility is maintained in conformity with state fire marshal regulations. Smoke detectors and carbon monoxide detectors functioning. Each client is accorded safe, healthful, and comfortable accommodations, furnishings and equipment to meet his/her needs. Snacks and emergency food are available. Disinfectants, cleaning solutions and poisons are inaccessible to individuals. Facility has adequate emergency supplies and first aid supplies. Facility temperature was observed to be 72 degrees. Outdoor walkways are free from obstruction and the facility has outside areas for individuals to use. Centrally stored medication is kept locked and inaccessible to clients. Medications are given as prescribed. LPA observed sufficient staff for client ratio. Emergency drills are conducted in accordance with regulations standards. Client and Staff files reviewed reviled no issues. At this time LPA noted no violations or citations as a result of the facility annual inspection,
