Facility Evaluation Report

At 7:30am on 04/15/2026, Licensing Program Analyst (LPA) Jeffries arrived to the facility and knocked on door and rang doorbell to no answered. LPA noted that on the door of the facility was a posted notice of extermination, building is safe to reenter on 04/09/2026. LPA noted that the window was partially unobstructed in the south east bedroom and LPA viewed a completely empty bedroom. With the observation of the extermination notice, no answer to the door knock and electronic door bell, and the observed empty bedroom, shows some evidence that the facility no longer has any clients. Additionally, LPA did a second visit at 1:30pm with no changes to the observations above. LPA will advise LPM on the closure of the facility.
