Facility Evaluation Report
At 7:41am on 06/04/2026, Licensing Program Analyst (LPA) Jeffries arrived at the facility to conduct and unannounced annual facility inspection. LPA asked direct care staff to call house Administrator, Alan Vaca to come the facility for the facility annual inspection. LPA met with Administrator, Alan Vaca announced who he is and the reason for the visit.
Administrator and LPA took a cursory tour of the facility. This facility is 4 bedroom, 2 bath room, living room kitchen, with a laundry area in the garage and extra food storage area, PPE storage, There is a living room and a large table/island separating the kitchen from the living room. There is a staff office and meeting room off the living room. Medications and files are locked and located in filing cabinets in the staff office/meeting room. LPA noted linin storage in the hallway closet. There is a back yard for clients to be outside and a covered area for shade. LPA observed 7 days of perishable and 2 days of non-perishable foods. LPA observed a working fire extinguisher under the kitchen sink, primed in the green as good.. LPA noted that each client room had furniture and linin within regulation requirements. LPA noted that both bathroom has paper towels and liquid soap. LPA noted that the facility was clean and in good repair and all hallways and exits were free and clear of obstacles. LPA reviewed Emergency Disaster Plan, Infection Control Plan, Centrally Stored Medication Records and Staff and Client files. LPA noted that 3 of 5 clients in care are over the age of 60 and Administrator was advised to submit an exception for retaining clients over the age of 60 that exceeds the 50% of clients in care per regulation 85068.4(g). At this time there were no violations or citations issued as a result of the annual facility physical inspection.
LPA and Administrator conducted the full review of annual care tools modules. LPA noted that there were no violations or citations as a result of the annual facility inspection.
