Facility Evaluation Report

Licensing Program Analyst (LPA) Melisa Rankin conducted an unannounced Case Management Visit and was joined by Tri-County Regional Center (TCRC) Quality Assurance Specialist (QAS) Wesley Marking. LPA and QAS met with Elizabeth Gomez, Director of Case Management, to discuss the purpose of the visit. Around 11:10 am, Administrator Josefina Sarabia arrived. Case Management visit was made due to an Incident Report submitted by the facility on 5/27/26.

Incident report documented there are allegations of 2 staff violating a clients personal rights. Administration stated staff members in question have been placed on administrative leave pending an internal investigation. Copy of schedule was obtained showing staff being investigated are not scheduled at this time. Other relevant documents were collected or were emailed to LPA during visit.

QA and LPA conducted staff interview and administrative interviews. LPA conducted facility tour. LPA noted administrative action, staff scheduling adjustments and staff care and plans for client and for staff support training. LPA requested findings of their internal investigation and documentation of any training conducted for all staff. LPA will review documentation and follow up with the facility if any questions. Plans for updated Crisis Prevention support have been and continue to be reviewed.

Based on interviews and records review, no immediate or potential health and safety concerns were observed during the visit and no deficiencies cited at this time. LPA has determined should further investigation be warranted, LPA will return at a later date.

Exit interview conducted. A copy of the report was issued.
