Facility Evaluation Report

Licensing Program Analyst (LPA) Rankin returned to finish the required unannounced 1 year annual visit to the facility above on 04/4/2025 around 1:45 pm. LPA met with the Administrator, Josie Sarabia, explained the purpose of the visit.

During last visit LPA was unable to complete a review of the Personnel Records, and needed to complete staff interviews.

Personnel Records & Training: The facility employes 5 full time staff and 1 administrator. Staff records are kept confidential and maintained at the main office. LPA reviewed 5 staff files. Files reviewed had current 1st Aid/CPR certifications,as well as all other required personnel documents. Administrator Certificate expires 3/25/26. Annual training was documented and complete for all staff reviewed.
