The inspector’s account

At 7:30am on 10/13/2025, Licensing Program Analyst (LPA) Jeffries arrived to the facility unannounced to conduct the annual facility inspection. LPA met with Administrator Edward Maldonado, announced who he is and the reason for the visit.

This facility is four bedroom all single client occupancy, two bathroom, living room, kitchen, dining room, and an office activity room. Medications are in a locked closet in the dining room. There is a complete first aide kit also located in the locked closet. There is a gated back yard that warps around to the gated front yard with table, chairs, and umbrella for shade. LPA observed three fire extinguishers placed throughout the facility that were all primed and reading in the green. LPA tested the water throughout the facility and noted that the water temperature was within regulations requirements of 105*-120*(f). LPA observed client bedrooms and noted that all rooms have appropriate furniture and linin per regulation requirements. LPA observed paper towels and liquid soap in both bathrooms. LPA observed at least two day of perishable foods and at least seven days of non-perishable foods on hand for four clients and staff. LPA observed emergency water supply in the garage. LPA observed and tested dual smoke and carbon monoxide detectors in all rooms and throughout the facility. LPA noted that all exits were free and clear of obstruction and the facility was clean and in good repair. Administrator noted at the end of the inspection that an additional door way was added to the Master bedroom. LPA will follow up with new fire clearance inspection request due to additional door. LPA reviewed staff files, including training hours, client files and centrally stored medication records (CSMR). LPA reviewed Infection Control Plan and Emergency Disaster Plan, and emergency drill logs. LPA noted that a full review of the annual care tools was also conducted during this annual facility inspection. LPA noted that there are no violations or citations as a result of this annual facility inspection.
