The inspector’s account

Licensing Program Analyst (LPA) Melisa Rankin conducted an unannounced Case Management - Other visit to the above facility. The purpose of today's visit to ensure an excluded individual is not employed at the facility. LPA met with Designee and explained the reason for the visit. LPA also spoke with Administrator Estella Usher over the phone, explaining the purpose of the visit.

During today's visit, the LPA inquired as to whether Staff #1 (S1) is currently employed at the facility. The Administrator via a phone call confirmed that S1 has not worked here for 2 or more years. LPA asked 3 other staff currently on site, 2 staff stated S1 has not been here for years, 1 stated, they didn’t even know who that was due to them being a newer employee. The Woodland Hills North Regional Office reviewed the legal case and the individual’s history, and the Stipulation/Decision and Order excluded the individual for life. The individual is not eligible to be licensed, reside or work in a facility. The Decision and Order was served to the licensee and S1 via certified mail on 1/15/2025 and was effective as of 01/27/2025. The Administrator was not aware of the Decision/Order, but LPA noted that the mail was sent to the Licensee’s address located in another city.

In addition, LPA obtained copies of the staff roster to verify that S1 is not employed at the facility.
