Facility Evaluation Report
At 8:57 am on 04/30/2026, Licensing Program Analyst (LPA) Rankin arrived unannounced to the facility to conduct the annual facility inspection. LPA met with the Administrator, Sarita Hidalgo, and explained the purpose of the visit.
A tour of the inside and outside of the facility was conducted, the following was inspected and noted during the annual visit:
Physical Plant & Environment Safety: This facility is a 3-bedroom, 3-bathroom residence with open living room setting, with kitchen area, den and staff office in common area. Facility occupies 4 residents and employs 13 staff and an administrator. The facility is clean, safe, and sanitary. The facility has smoke and carbon monoxide detectors which were tested and working during time of visit. The lighting in common areas was sufficient. Toilet, hand washing and bathing facilities are operational. The pathways are clear of any obstructions. Disinfectant, cleaning solutions and poisons are in a locked cabinet under the kitchen sink cabinet. The facility has sufficient space inside and outside for activities and visiting. The facility has an enclosed backyard for client use. Water temperature was within regulation requirements and facility checks and keeps a log of temperatures.
Operational Requirements: The Facility is operating in compliance with the granted fire clearance. The facility is approved for a capacity of 4 Ambulatory of which 3 can be non-ambulatory.
Staffing, Personnel and Training Records: The facility employes 13 staff and 1 Administrator. LPA reviewed 4 staff files, and all required documents were present. All staff reviewed had current 1st Aid/CPR along with required annual training. Administrator’s certificate expires 4/13/27.
