Facility Evaluation Report
At 9:45am on 04/15/2026, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility annual inspection. LPA met with Assistant Program Director, Sheryl Sterling (S1) announced who he is and the reason for the visit.
S1 and LPA conducted a facility physical walking tour. LPA noted that the facility is set up in a square collection of attached buildings. There are 10 classrooms, 2 kitchens, 4 client bathrooms (set up for non ambulatory clients), a large enclosed outdoor quad for clients out door activities. The main entrance has assorted offices, conference room, and utility storage room, and 2 all purpose bathrooms. LPA noted that there were fire extinguisher located throughout the facility that were all primed and in the green. LPA noted that each kitchen has carbon monoxide detectors, which was tested and functioning. LPA noted that the buildings (all rooms) have internal fire extinguishers that were recently serviced by Whittle Fire on February 3rd, 2026. LPA noted that the classrooms and kitchens were free and clear of obstacles and debit. LPA noted that required posting were located at the administration office doorway entrance. LPA conducted a file review of staff and client files. LPA advised S1 that loading and unloading of clients should be monitored for safety in the driveway in front of the facility. All other staff and client files were complete during review. LPA noted that in 2025 the facility started providing lunches from a company related kitchen located on same property but not licensed. LPA conducted a full review of the annual care tools and found no violations. As a result of this annual inspection, there were no violations or citations issued.
