Facility Evaluation Report
At 8:00am on 04/28/2026, Licensing Program Analyst (LPA) Jeffries arrived unannounced to the facility to conduct the facility annual inspection. LPA called facility Administrator, Clinton Cadle, announced who he is and the reason for the visit, Mr. Cadel was unable to be at the facility due to medical reasons, and provided permission for Staff Lori Torres (S1) to go through the annual inspection process and sign the annual report. .
S1 and LPA conducted a full physical tour of the facility both inside and outside. This is a 4 bedroom, two bathroom, living room/office, dining area, and kitchen, with a laundry room. Client rooms are all single occupancy and are set up per regulations requirements. The back yard has a table, chairs and umbrella for clients and visitors. Medications and confidential files are locked in cabinet and drawers between the dining area and living room. LPA noted that there are at least 2 days of perishable foods and 7 days of non-perishable foods on hand for 4 clients and staff. LPA noted that the facility is clean and in good repair. LPA observed a fire extinguisher located at the entrance hallway, primed in the green reading. LPA observed and tested smoke detectors and carbon monoxide detectors. LPA observed liquid soap and paper towels in the bathrooms. LPA noted that emergency water supply in located in the garage area. LPA noted all posting to be in the hallway poster board area. LPA noted that the first aide kit is complete and in the medication closet.
LPA conducted a full review of the annual care tools modules. LPA conducted client file reviews, with no issues. LPA reviewed emergency disaster plan, reviewed and signed by Staff as reviewed. LPA reviewed infection control plan reviewed and signed by Staff. LPA reviewed all staff to be active on roster fingerprinted and cleared to work at this facility. LPA noted that there were no violations or citations issued during this annual facility inspection. Exit interview, report read, and report provided.
